1.- Original Title
2.- Bill of Sale
3.- Pictures of vehicle at time of purchase at the auction
4- Invoices of the parts that were replaced in the vehicle
5.- Power of Attorney (if applies)
Fill in and sign:
6.- (form) HSMV 84490
7.- (form) HSMV 84491
8.- (form) HSMV 82040
Personal / Individual:
1.-Original Title
2.-Bill of Sale from Insurance Co (only if title is not under your name as registered owner)
3.- Damage Report (from Insurance Co.)
4.-Pictures of vehicle before and after damage/repairs
5.-Invoice of any Major Component parts replaced
6.-BodyShop Order (if applies)
7.-Valid FL Driver’s License
